Step 3 - Personalized Protocol

If TRT is clinically appropriate, a protocol is prescribed based on your labs, symptoms, and lifestyle. How much does TRT cost at Steel City HRT?

The initial consultation fee depends on which initiation track you select. Standard TRT membership for injectable testosterone is $180 per 4-week cycle, which includes medication, shipping, and clinical oversight. High-dose scrotal cream is $215 per 4-week cycle and an oral testosterone capsule is $245 per 4-week cycle. A 10% discount applies for verified military, first responders, and healthcare workers on the standard injection track

Does TRT cause hair loss?

Hair loss on TRT is driven by genetic predisposition to DHT sensitivity — not TRT itself. In men who carry that pattern, higher androgen levels can accelerate a process that was already programmed into their follicles. It does not cause hair loss in men who are not genetically predisposed. For men who are concerned or already noticing early thinning, there are proactive management options we can work into your protocol. We handle it in-house — no separate dermatologist, no second appointment.

Does TRT cause acne?

Some men experience breakouts, especially in the first few weeks as androgen levels shift. It is more common in men who had acne-prone skin before starting. Most of the time it resolves on its own once levels stabilize. When it does not, there are treatment options we can add directly through your Steel City protocol — addressed at the same telehealth visit without a separate referral.

Does TRT affect prostate cancer risk?

Properly monitored TRT has not been shown to cause prostate cancer in men without pre-existing prostate pathology. A large clinical trial reviewed by the FDA concluded that testosterone therapy did not increase cardiovascular risk, and the evidence on prostate safety has similarly moved in a more favorable direction. PSA is measured at baseline and monitored quarterly at Steel City. TRT is not initiated in men with active or suspected prostate cancer — candidacy is evaluated individually at your consultation.

Does tadalafil come with my TRT protocol?

All active TRT members at Steel City have access to daily low-dose tadalafil as part of their protocol when clinically appropriate. Daily low-dose tadalafil at 5 mg is showing measurable benefits across multiple systems — circulation, nitric oxide signaling, exercise performance, and sexual function — which is why it pairs well with TRT rather than sitting in a separate silo. It is prescribed at your initial consultation or any follow-up visit. Men with known contraindications such as nitrate use are evaluated individually.

Is TRT safe for my heart?

Yes, for appropriate candidates under proper monitoring. In February 2025, the FDA removed the black box cardiovascular warning from all testosterone products after reviewing a large clinical trial that found no increased risk of heart attack or stroke. A growing body of evidence actually shows that low testosterone itself is associated with higher cardiovascular risk. Steel City does not initiate TRT in men with a cardiac event within the prior six months. Hematocrit, PSA, and hormone panels are tracked quarterly to stay ahead of any emerging issues.

Will I have to stay on TRT forever?

Most men do stay on it long-term because TRT manages the condition rather than curing it — if you stop, levels return to where they were. That said, some men choose to come off, and we support that with a proper taper and transition plan. Men who want to preserve the option to stop — or who are not ready to commit long-term — are often good candidates for enclomiphene, which stimulates your own production rather than replacing it externally.

Can TRT help with weight loss?

Indirectly, yes. Testosterone plays a direct role in muscle protein synthesis and fat metabolism. Men on optimized TRT with consistent training and adequate protein consistently see body composition shifts — more lean mass, less fat — beginning around 8–12 weeks. It is not a standalone weight loss therapy. What happens if I stop TRT?

Stopping TRT abruptly causes testosterone to drop back toward your pre-treatment baseline. Some men experience a period of low energy, mood changes, and reduced libido while the body recalibrates. If maintaining natural production matters to you — for fertility reasons or to keep the option to stop open — HCG support or enclomiphene protocols can be incorporated from the start.

Does TRT affect sleep or cause sleep apnea?

TRT can worsen existing sleep apnea in men who are already predisposed to it — this is a known and monitored parameter, not a reason to avoid treatment. If you have a diagnosed sleep disorder or symptoms like loud snoring, gasping, or unrefreshing sleep, that gets flagged at your initial consultation and factored into your protocol. Can I drink alcohol on TRT?

Moderate alcohol use is not a hard contraindication to TRT, but it works against your protocol in a few meaningful ways. Alcohol suppresses testosterone production, increases aromatization of testosterone to estrogen, disrupts sleep architecture, and impairs the muscle protein synthesis that makes TRT actually deliver results. Occasional drinks are not going to derail your progress. Regular heavy drinking will. What is estradiol and why does it matter on TRT?

Estradiol is the primary estrogen in men, and it is produced when testosterone converts to estrogen through a process called aromatization. On TRT, as testosterone levels rise, estradiol can rise with it. The right amount of estradiol is actually essential — it protects bone density, supports cardiovascular health, regulates libido, and contributes to erectile function. Too much causes water retention, moodiness, and sexual dysfunction. Too little — which happens when estrogen is over-suppressed — causes joint pain, low libido, and brain fog. At Steel City, estradiol is monitored at every lab interval and managed with an aromatase inhibitor only when levels warrant it. We do not suppress estrogen reflexively — we manage it to range.

Does TRT affect mood or cause aggression?

The "roid rage" picture most men have in their heads comes from supraphysiologic doses — amounts used in performance contexts that are multiples above what any legitimate TRT protocol prescribes. At therapeutic doses calibrated to restore normal physiologic levels, TRT consistently improves mood stability, reduces irritability, and lowers rates of depressive symptoms.
