1. Nature of Services

Steel City HRT and Weight Loss provides clinician-led medical services, which may include:

Hormone Replacement Therapy (HRT)

Testosterone Replacement Therapy (TRT)

Medical weight management

Nutritional and wellness counseling

Peptide therapies

Telehealth consultations

All care is individualized and provided by licensed medical professionals. Treatment decisions are based on medical history, symptoms, laboratory data, and provider's clinical judgment.

Participation is voluntary. Alternatives, including no treatment, are available.

Certain treatments may involve off-label use of FDA-approved medications when clinically appropriate.

2. No Guarantee of Treatment or Prescription

Payment for services does not guarantee that any medication will be prescribed.
Prescriptions are issued only after medical evaluation and solely at the provider’s discretion.

3. Financial Terms

The Clinic operates on a cash-based model and does not bill insurance.

Payment is due at the time of service unless otherwise disclosed.

Memberships or subscription services may involve recurring charges every four (4) weeks.

Cancellation or billing changes require written notice at least seven (7) days prior to the next billing date.

Fees are for professional medical services, care coordination, and monitoring — not for the sale or dispensing of medications.

No refunds are issued for services already rendered except as required by law.

4. Telehealth Services

Telehealth services may be offered when clinically appropriate. Telehealth is not suitable for emergencies. If you are experiencing a medical emergency, call 911 or seek immediate care.

Prescribing via telehealth complies with all applicable state and federal laws.

5. Limitation of Liability

The Clinic is not liable for outcomes resulting from incomplete or inaccurate patient information, non-compliance with medical recommendations, or misuse of prescribed medications.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or fraud.

6. Governing Law

These Terms are governed by the laws of the State of Colorado.
